Monitor patients' health status by observing vital signs such as blood pressure, pulse, and temperature; report changes to healthcare professionals promptly. Support medication administration following care plans and ensuring compliance with HIPAA regulations to protect patient confidentiality. Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary. Use proper manual handling techniques and patient transfer methods, including Hoyer lifts and other assistive devices, to ensure safety during movement. Maintain accurate documentation of care provided, patient observations, and updates to care plans; communicate effectively with clients and team members. Support patients with cognitive impairments such as dementia or Alzheimer's disease through memory care techniques and personalized engagement strategies. Qualifications Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly desirable. Demonstrated ability to observe patients closely for changes in condition or behavior; experience with patient monitoring is essential. Knowledge of HIPAA regulations and commitment to maintaining client confidentiality at all times. Certification in CPR and First Aid is required; additional certifications in hospice care, dementia care, or neurodevelopmental disorders are advantageous. Experience working with individuals with autism, developmental disabilities, or intellectual disabilities supports success in this role. Ability to perform heavy lifting safely and utilize patient transfer methods effectively; manual handling skills are crucial.
