Job Responsibilities Assist clients in their homes with daily living activities including personal care and grooming, bathing, dressing, toileting, hair care, and skincare Assist patients with walking, transferring in and out of wheelchairs, and changing positions in bed STNA/Home Health Aide provides patients with companionship and emotional support Assist with needs around the home Document services provided in the client's electronic medical chart Work in cooperation with our caring home health and hospice team Adhere to professional standards, follow policies and procedures, and abide by federal, state, and agency requirements as well as quality accreditation standards Qualifications and Skills High school diploma or equivalent Physically capable to lift and transfer patients Excellent communication skills, including the ability to communicate sensitive information with empathy ("bedside manner") Ability to work as part of a team and handle multiple tasks safely and effectively Reliable transportation, current driver's license, and insurance About CHP CHP is a local, nonprofit organization and one of the region's most trusted and established providers.
