Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patient health by observing vital signs such as blood pressure, pulse, temperature, and respiration; report changes promptly Support medication administration following prescribed protocols while maintaining strict adherence to HIPAA and home healthcare regulations Provide specialized care for clients with dementia, Alzheimer's disease, or other memory impairments; assist with memory care strategies Operate Electronic Health Record (EHR) systems to document patient progress accurately and maintain medical records securely Facilitate safe patient transfers using proper methods such as Hoyer lifts or transfer belts; ensure patient safety at all times Assist with light housekeeping tasks including cleaning, laundry, and meal preparation to promote a healthy living environment Support end-of-life care with compassion and respect for patient dignity; communicate effectively with family members and healthcare teams Maintain patient confidentiality diligently while providing high-quality home health care assistance tailored to individual needs Use medical terminology appropriately when documenting or communicating health information with other professionals Qualifications Proven experience working as a Home Health Aide or in assisted living/nursing home settings; experience with individuals with disabilities or developmental delays is highly valued Knowledge of medical terminology, medication administration, catheter care, and vital signs monitoring Certification in First Aid and Basic Life Support (BLS), including CPR training; ability to respond swiftly in emergency situations Familiarity with EMR/EHR systems for accurate documentation; experience working within home health agency regulations is preferred Strong communication skills for effective client interaction and team collaboration Ability to lift heavy objects safely using proper patient transfer methods; physical stamina for long shifts is essential Experience providing dementia care, hospice support, or geriatrics is advantageous Demonstrated commitment to maintaining patient confidentiality in accordance with HIPAA standards Valid driver's license and reliable transportation are often required for travel between client homes Join us in delivering exceptional home health care that enriches lives through compassionate service.
