Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor patient health by observing vital signs such as blood pressure, pulse, respiration, and temperature Administer medications and perform basic medical procedures following healthcare provider instructions and home health regulations Maintain accurate medical documentation using Electronic Health Record (EHR) systems and ensure compliance with HIPAA to protect patient confidentiality Support patients with specialized needs such as Alzheimer's care, dementia care, end-of-life care, and memory care Use patient transfer methods safely, including Hoyer lifts and other assistive devices, to prevent injury during mobility assistance Perform light housekeeping tasks like cleaning, laundry, meal preparation, and ensuring a safe environment for the patient Experience Prior experience working in assisted living facilities, nursing homes, or home healthcare settings preferred Demonstrated ability to work with individuals with developmental disabilities or disabilities requiring specialized care Knowledge of medical terminology, catheter care, medication administration, and vital signs monitoring essential Experience in hospice or geriatrics care highly valued Familiarity with home health agency operations and home & community care regulations is advantageous Certification in First Aid and Basic Life Support (BLS), CPR required or obtainable upon hire Strong communication skills for effective client interaction and team collaboration Join us if you're passionate about providing compassionate patient-centered care!
