Overview We are seeking compassionate and dedicated Home Health Aides to provide essential care and support to individuals in their homes. As a Home Health Aide, you will play a vital role in enhancing the quality of life for our clients, assisting them with daily activities, and ensuring their comfort and safety. This position requires a strong commitment to patient care and the ability to work independently while following care plans. Responsibilities Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and meal preparation. Monitor and document vital signs, including temperature, pulse, and blood pressure. Provide companionship and emotional support to clients, fostering a positive environment. Administer medications as prescribed and ensure adherence to medication schedules. Assist with mobility using equipment such as Hoyer lifts when necessary. Maintain cleanliness in the client's living space, including laundry and light housekeeping tasks. Observe and report any changes in the client's condition or behavior to the healthcare team. Follow HIPAA regulations to ensure client confidentiality and privacy. Support clients with memory care needs, including those with Alzheimer's or dementia. Collaborate with family members and other healthcare professionals to provide comprehensive care. Skills Strong patient observation skills to identify changes in health status. Experience in assisted living or nursing home environments is preferred. Familiarity with EMR/EHR systems for accurate medical documentation. Knowledge of medical terminology related to patient care. CPR certification is highly desirable. Experience with catheter care and hospice care is a plus. Ability to work effectively with individuals who have developmental disabilities or other special needs. Proficient in providing senior care, including home health and long-term care experience. Capable of performing cleaning duties related to client care environments. Excellent communication skills for interacting with clients, families, and healthcare teams.
