You will be responsible for assisting with activities of daily living (ADLs), monitoring patient health, and maintaining accurate medical documentation—all while adhering to home health care regulations and ensuring patient confidentiality. Duties Observe and monitor patients' conditions regularly to detect changes or concerns early Assist with activities of daily living such as bathing, dressing, grooming, and mobility support Administer medications and manage medical treatments following prescribed protocols Record vital signs accurately and report findings to supervising healthcare professionals Support patients with Alzheimer's, dementia, or memory care needs through specialized techniques Operate EMR (Electronic Medical Records) systems to document patient information securely and efficiently Use patient transfer methods safely, including Hoyer lifts and other assistive devices Provide end-of-life care with compassion and respect for patient dignity Assist with meal preparation, light housekeeping, laundry, and cleaning to maintain a safe environment Follow HIPAA guidelines diligently to maintain patient confidentiality at all times Requirements Proven experience working with individuals with developmental disabilities or in geriatrics and senior care environments Knowledge of medical terminology, catheter care, and medication administration procedures Familiarity with EMR/EHR systems for accurate medical documentation Certification in CPR (Cardiopulmonary Resuscitation) and basic life support (BLS) is required Experience working in assisted living facilities, nursing homes, or home healthcare agencies is highly preferred Ability to lift heavy objects safely and perform physical tasks such as patient transfers using appropriate methods Strong communication skills for effective client interaction and team collaboration Understanding of home health care regulations and best practices in hospice or palliative care Commitment to maintaining patient safety, privacy (HIPAA compliance), and confidentiality Flexibility to work live-in shifts or varied schedules as needed Join us if you're passionate about providing energetic, high-quality patient care that transforms lives!
