Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor patient health by observing vital signs such as blood pressure, pulse, temperature, and respiration; document findings accurately using Electronic Health Record (EHR) systems Support medication administration following prescribed protocols while maintaining strict adherence to home health care regulations and HIPAA confidentiality standards Provide specialized care for patients with dementia or Alzheimer's disease, including memory care techniques and behavioral management strategies Perform catheter care and other medical procedures such as wound dressing changes or basic medical documentation as directed by healthcare providers Facilitate patient transfers using appropriate methods like Hoyer lifts or transfer belts; ensure safety during movement and repositioning Maintain a clean, safe environment by assisting with light housekeeping tasks such as laundry, cleaning, meal preparation, and ensuring proper infection control practices are followed Skills Proven experience working with geriatric patients or individuals with disabilities in assisted living or long-term care settings Strong knowledge of medical terminology, patient care procedures, and home health regulations Ability to operate EMR/EHR systems efficiently for documentation and communication purposes Excellent patient observation skills to detect changes in condition or behavior promptly Proficiency in assisting with ADLs and providing personal care assistance with compassion and professionalism Knowledge of CPR and First Aid certification; ability to respond effectively during emergencies or end-of-life situations Physical stamina for heavy lifting tasks and the ability to perform patient transfers safely using appropriate techniques and equipment Join us if you're passionate about delivering exceptional home health care assistance!
