Whether supporting residents in group homes, assisted living facilities, or community settings, your positive attitude and attentive care will foster a nurturing environment where clients feel valued and empowered. Duties Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and personal hygiene to ensure comfort and dignity. Monitor patient health by observing vital signs such as blood pressure, pulse, and temperature; report any changes or concerns promptly. Support medication administration following prescribed care plans while adhering to HIPAA regulations to maintain confidentiality. Facilitate patient transfers using manual handling techniques and specialized equipment like Hoyer lifts to ensure safety for both clients and staff. Prepare nutritious meals and assist with meal planning, ensuring dietary needs are met for individuals with specific health conditions such as dementia or allergies. Engage clients through meaningful activities that promote cognitive stimulation and social interaction, including memory care and autism support initiatives. Maintain a clean and safe environment by performing routine cleaning tasks, ensuring compliance with health standards in line with home health and hospice care practices. Document care activities accurately in client records, including progress notes, medication logs, and incident reports to support comprehensive care documentation. Skills Proven experience working with individuals with developmental disabilities, intellectual disabilities, or neurodevelopmental disorders such as autism. Strong patient observation skills to detect changes in condition or behavior promptly. Knowledge of dementia care, Alzheimer's care, memory care strategies, and geriatrics to provide specialized support. Familiarity with patient monitoring techniques including vital signs measurement and patient transfer methods like Hoyer lifts. Ability to perform CPR and First Aid confidently to respond effectively during emergencies. Excellent communication skills for engaging with clients, families, and healthcare teams while respecting privacy laws like HIPAA. Experience in home & community care environments such as nursing homes or assisted living facilities is highly desirable. Physical stamina for heavy lifting, manual handling, assisting with mobility, and supporting patients during transfers or activities requiring physical effort.
