Full Job Description Essential Duties and Responsibilities :
Provide direct care and supervision to individuals with developmental disabilities. Assist with activities of daily living (ADLs), including personal hygiene, bathing, dressing, grooming, toileting, and meal preparation. Administer medications as trained and authorized. Assist individuals with mobility, transfers, and use of adaptive equipment as needed. Support individuals in developing and maintaining independent living skills. Accompany and transport individuals to appointments, recreational activities, community outings, and social events. Implement Individual Service Plans (ISPs) and document progress toward goals. Complete accurate and timely documentation, including daily notes, medication administration records, and incident reports. Maintain a safe, clean, and supportive environment. Monitor health and behavioral concerns and report changes to supervisors and appropriate team members. Promote dignity, choice, self-advocacy, and person-centered care. Follow all DODD, county board, agency, and state regulations and policies.
