About the Role As a Hospice Care Assistant (CNA), you'll play an important role in supporting our mission of providing exceptional care and service. Whether working directly with patients, supporting operations, or partnering across departments, you'll have the opportunity to contribute in meaningful ways while continuing to grow your career. ❤️ What You'll Do 🌿 Provide Compassionate Patient Care Assist patients with activities of daily living, including bathing, grooming, dressing, toileting, and personal hygiene. Provide assistance with positioning, transfers, ambulation, and mobility needs. Offer companionship and emotional support to patients and families. Respect each patient's dignity, privacy, preferences, and individual routines. Encourage independence and provide care in a way that promotes comfort and confidence. ❤️ Promote Comfort & Quality of Life Provide comfort measures according to the patient's individualized plan of care. Observe patients for changes in condition, behavior, pain, or comfort level and promptly communicate concerns to the supervising nurse. Assist with creating a peaceful, safe, and comfortable environment. Provide compassionate presence and reassurance to patients and loved ones throughout the end-of-life journey. 🤝 Communicate & Collaborate Work closely with the RN case manager and interdisciplinary hospice team to support individualized patient care. Communicate changes in patient condition or needs to the appropriate member of the clinical team. Build trusting relationships with patients, families, caregivers, and members of the hospice team. Follow the patient's plan of care and perform duties within the scope of your certification and role. 🏡 Support a Safe & Comfortable Environment Assist with maintaining a clean, organized, and safe patient environment. Change linens and assist with basic household tasks related to patient care as appropriate. Follow infection prevention, safety, and patient privacy standards. Complete accurate and timely documentation of care provided. Required Qualifications Current CNA certification in the state of Ohio. Minimum of one year of experience working as a CNA in hospice, palliative care, or a similar healthcare setting. Valid driver's license and reliable transportation. Clean driving record and current auto insurance. Ability to travel throughout the assigned service area. Strong communication, teamwork, and interpersonal skills. Compassionate, patient-focused approach to care. We'd Love to Meet Someone Who Is Dependable, compassionate, and patient. Comfortable providing hands-on care in patients' homes.
