About the Role As a Care Consultant , you'll be a trusted connection between our hospice team and the healthcare professionals, patients, and families we serve. You'll build meaningful referral partnerships, educate healthcare professionals about the hospice benefit, and help communities better understand the compassionate care and support hospice can provide. Through genuine relationship-building, community outreach, and a heart for helping others, you'll play an important role in connecting patients and families with the care they deserve during one of life's most meaningful moments. What You'll Do Build Meaningful Relationships Develop and strengthen relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, home health agencies, and other referral partners. Serve as a trusted resource by educating healthcare professionals on hospice eligibility, services, and the benefits of early hospice involvement. Build long-term partnerships founded on trust, collaboration, and exceptional service. Drive Community Growth Develop strategic territory plans to increase awareness of Cadre Hospice throughout the community. Generate qualified patient referrals through consultative sales and relationship-building techniques. Identify new opportunities to expand Cadre's presence and strengthen referral networks. Partner with branch leadership to support sustainable growth and increased patient access. Educate & Advocate Represent Cadre Hospice at community events, educational programs, and networking opportunities. Provide education to referral partners, patients, caregivers, and community organizations regarding hospice and end-of-life services. Advocate for earlier access to hospice care while helping families better understand their options. Collaborate with Your Team Work closely with clinical and operational leaders to ensure a seamless referral and admission experience. Maintain regular communication regarding referral activity, market trends, and growth opportunities. Support branch initiatives while contributing to an exceptional patient and referral partner experience. Required Qualifications Minimum of two (2) years of healthcare marketing, referral development, business development, or healthcare sales experience. Strong understanding of consultative sales and relationship-building strategies. Knowledge of hospice, palliative care, post-acute care, or related healthcare services. Valid driver's license, reliable transportation, and a clean driving record. Excellent communication, presentation, and interpersonal skills. Preferred Qualifications Hospice or home health business development experience. Established relationships within the local healthcare community. Ability to work independently while collaborating effectively with interdisciplinary teams. Passion for community outreach and improving access to compassionate end-of-life care. Why Join Cadre?
