Job Overview We are seeking compassionate and energetic Home Health Aides to join our team. In this vital role, you will provide personalized in-home care to patients in private residences. Your dedication will help improve quality of life for individuals with diverse needs, from elderly patients to those with developmental disabilities or memory care requirements. This paid position offers an opportunity to make a meaningful difference every day by supporting patients with their daily activities and health needs. We are looking for someone who can work evenings from 4pm-6pm Duties Assist patients with activities of daily living (ADLs), such as bathing, dressing, grooming, and mobility support Monitor patient health by observing vital signs and reporting changes promptly Support medication administration and manage medical documentation accurately using Electronic Health Record (EHR) systems Provide specialized care for individuals with dementia, Alzheimer's, or other memory impairments Assist with catheter care, wound care, and other medical procedures following home healthcare regulations Help with meal preparation, light housekeeping, laundry, and maintaining a clean living environment Use patient transfer methods such as Hoyer lifts or transfer belts safely and effectively Qualifications Prior assisted living or home healthcare experience preferred; experience working with geriatric patients or individuals with disabilities highly valued Knowledge of medical terminology, vital signs monitoring, medication administration, and basic life support (CPR/First Aid) Familiarity with EMR/EHR systems for medical documentation and record keeping Ability to work independently while maintaining excellent client communication and confidentiality in accordance with HIPAA standards Strong caregiving skills with a compassionate approach to patient personal care assistance Physical ability to perform heavy lifting and assist with patient transfers safely Experience working within home health agency regulations and providing hospice or end-of-life care is a plus Join us in making a positive impact on our patients' lives through dedicated home health care assistance.
