Position Summary The MSW provides comprehensive psychosocial assessments and support services to patients and families, collaborating closely with the interdisciplinary team to ensure holistic, patient-centered care. Key Responsibilities Conduct comprehensive psychosocial evaluations related to the patient's illness and home environment Participate in the development and implementation of the patient's individualized care plan Collaborate with patients, families, caregivers, and the interdisciplinary team Identify needs for additional home health services and initiate referrals as appropriate Connect patients and families with relevant community resources Meet agency productivity expectations Qualifications Master's degree in Social Work from a program accredited by the Council on Social Work Education Minimum of 1 year of experience required Home Health experience strongly preferred Current CPR certification Valid Oregon driver's license and current auto insurance Reliable, insured vehicle in good working condition Strong critical thinking and communication skills Our Vision We believe in a holistic approach to care that values each person's physical, emotional, and spiritual needs.
