Job Summary Join our dedicated healthcare team as a Home Health Aide, where you will play a vital role in providing compassionate, personalized care to individuals in their own homes. As a Home Health Aide, you will work closely with patients, families, and healthcare professionals to deliver high-quality home health care that promotes independence and enhances quality of life. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and observe patient conditions, recording vital signs and any changes in health status accurately in electronic health record (EHR) or electronic medical record (EMR) systems Provide medication reminders and assist with medication administration following medical instructions and home healthcare regulations Support patients with specialized needs such as Alzheimer's care, dementia care, memory care, and end-of-life care with patience and empathy Perform catheter care, wound care, and other medical procedures as trained and authorized Help with light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe environment for clients Utilize patient transfer methods such as Hoyer lifts or patient transfer techniques to ensure safe mobility assistance Maintain strict confidentiality of patient information in compliance with HIPAA regulations and uphold patient rights at all times Communicate effectively with clients and family members to address concerns and provide updates on patient status Follow all home health agency protocols and adhere to state-specific regulations for home healthcare services Experience Prior experience working in assisted living facilities, nursing homes, or home healthcare environments preferred Demonstrated experience working with individuals with developmental disabilities or geriatrics required Knowledge of medical terminology, vital signs measurement, CPR certification, first aid training, and basic life support (BLS) essential Experience with EMR/EHR systems for documentation of patient care activities highly desirable Familiarity with hospice care practices and heavy lifting techniques such as using Hoyer lifts preferred Proven ability to provide compassionate caregiving for clients with complex needs including Alzheimer's disease or other memory impairments Previous work involving client communication skills and maintaining patient confidentiality is necessary Join us in delivering exceptional home health care that empowers individuals to live safely and comfortably in their own homes.
