Key responsibilities include:

• Assist residents with their individual support care needs by carrying out duties and implementing their Individual Service Plans.
• Safely conduct recreational outings in the community with residents to promote quality of life and community integration, modeling appropriate social behavior while representing the agency.
• Encourage residents to engage in social and recreational activities to foster community integration and personal development.
• Assist residents with meal planning and preparation, ensuring adherence to prescribed dietary needs.
• Maintain resident laundry and assist with laundry tasks as necessary to ensure cleanliness and hygiene.
• Perform housekeeping duties such as dusting and vacuuming to maintain clean and sanitary living conditions.
• Provide mobility assistance, including travel within the community for shopping, medical appointments, recreational activities, school, and home visits.
• Administer medications to each resident according to doctors' orders and maintain accurate medication records.
• Ensure timely and thorough documentation that meets agency expectations.
• Assist residents with individual support care needs by coordinating with external providers such as schools and mental health professionals, aligning with goals outlined in the Individual Service Plan.
• Review and update records with reports from schools, therapists, and other providers to track progress or identify any setbacks.
• Coordinate services with schools, hospitals, child protective services, mental health providers, doctors, and other relevant entities to ensure comprehensive care for the youth.
• Support youth and their families in accessing and effectively using resources and services
• Conduct home visits to engage with, inform, and educate families, fostering collaboration.
• Address any health and safety concerns promptly.
• Accompany residents to medical appointments and ensure follow-through on professional health recommendations.
• Attend all required meetings pertinent to the assigned youth and other operational requirements.
• Ensure adherence to the agency's transportation, emergency preparedness, and response policies to guarantee the safety of all residents.
• Complete accurate, timely, and objective documentation such as progress notes, behavior charts, and medication administration records in compliance with all state and federal regulations.
• Uphold each individual's rights, maintain confidentiality, and promote self-esteem and decision-making skills.
• Protect residents from abuse and neglect by adhering to agency policies and state laws and reporting concerns as mandated.
• Notify supervisors immediately of emergencies when safe to do so.
• Perform all job assignments in compliance with local, state, and federal regulations and adhere strictly to agency policies and procedures.
• Undertake other duties as assigned.

Qualifications and Educational Requirements:

An Associate degree in human services, childhood development, psychology, or a related field is required; a Bachelor's degree in a related field is preferred. At least two (2) years of experience working with youth in a residential setting is preferred. Must complete background checks as mandated by state regulations. A valid driver's license and insured vehicle are required.

Physical Requirements:

Availability to work weekends and holidays. Ability to read, write, and communicate fluently in English. Must be capable of seeing to read labels, documents, and computer screens. Requires standing, moving about, or walking for extended periods; may involve kneeling, bending, or reaching. Dexterity of hands and fingers needed for keyboarding and other office tasks.
