Description Overview Join our dedicated healthcare team as a Home Health Aide and CNA Worker, where your compassionate care and attention to detail will make a meaningful difference in the lives of our clients. This rewarding role involves providing personalized assistance to individuals in their homes or assisted living facilities, ensuring their safety, comfort, and well-being. You will be an essential part of a supportive team committed to delivering high-quality patient care with professionalism and empathy. All positions are paid, reflecting the value we place on your skills and dedication. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts or transfer belts

Monitor and record vital signs, medical documentation, and patient observations accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems

Administer medications and perform catheter care following medical protocols and HIPAA regulations to ensure patient safety and confidentiality

Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments, including memory care techniques and behavioral management strategies

Support patients with hospice or palliative needs by delivering compassionate care aligned with their wishes and medical plans

Assist with meal preparation, feeding, light cleaning, laundry, and maintaining a safe, clean environment in accordance with health standards

Document all patient interactions thoroughly in medical records while adhering to privacy regulations and ensuring compliance with healthcare policies Qualifications Proven experience working in home health settings, nursing homes, assisted living facilities, or long-term care environments

Knowledge of medical terminology, medication administration, vital signs monitoring, and dementia or Alzheimer's care techniques

Certification as a CNA (Certified Nursing Assistant) or Home Health Aide is required; CPR certification is highly preferred

Experience working with individuals with developmental disabilities or disabilities requiring specialized support is a plus

Familiarity with EMR/EHR systems for accurate documentation and patient record management

Ability to lift patients safely using Hoyer lifts or other assistive devices; strong patient observation skills are essential
