Faithful Homes, LLC is looking for an experienced Direct Support Professional to join their team. This specific position would be working 1:1 with an individual with an intellectual/developmental disability in their home and community to support independence and ensure health and safety. Experience and comfortability working with individuals with behavioral challenges is required.

Key Responsibilities:

Providing direct care to individual(s) according to their individualized support plans (ISPs). Assist individuals in the development of appropriate life skills, necessary to reach their highest potential. Facilitating and supporting the individual(s) to participate in everyday activities, recreation and leisure. Medication administration Completion of daily documentation Transportation to/from outings and medical appointments. During ER visits and hospitalizations ensure all information pertaining to the individual is readily accessible to present to EMT's, nurses, doctors and social workers and ensure that managers are provided updates on status of individual and are contacted when the doctor comes into the room.

Qualifications:

Education

• High School Diploma or GED Age
• 18 years or older Ability to drive
• valid PA Driver's License Reliable Transportation Completion of Background checks as outlined per state regulations Health-Certification from a licensed independent practitioner that the candidate is free from communicable disease.
• Experience working with individuals with behavioral challenges is required
