Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor patient health by observing vital signs such as blood pressure, pulse, respiration, and temperature; report changes promptly to healthcare providers Support medication management by administering prescribed medications following medical instructions and maintaining accurate documentation Provide specialized care for patients with conditions such as Alzheimer's disease, dementia, or developmental disabilities, ensuring safety and comfort at all times Maintain detailed medical records using Electronic Health Record (EHR) systems while adhering to HIPAA regulations to protect patient confidentiality Assist with transfers using patient transfer methods like Hoyer lifts or other assistive devices to ensure safe mobility Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a clean and safe environment for the patient Experience Prior experience working in assisted living facilities, nursing homes, or home healthcare settings is highly valued Demonstrated ability to work with geriatric patients and individuals with disabilities or chronic conditions such as Alzheimer's or dementia Knowledge of medical terminology, home health care regulations, and documentation standards required for accurate medical records management Experience operating EMR (Electronic Medical Records) systems and maintaining patient privacy under HIPAA guidelines Certification in First Aid and Basic Life Support (BLS), including CPR training is preferred but not mandatory; willingness to obtain certification is acceptable Proven ability to handle physical tasks such as heavy lifting or assisting with patient transfers safely and effectively Strong communication skills for engaging with patients and collaborating with healthcare teams to deliver comprehensive care Join us if you're passionate about providing compassionate home health care that empowers individuals to live healthier and more independent lives.
