Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor and observe patients' health status, recording vital signs such as blood pressure, pulse, and temperature accurately Support medication administration following medical instructions and maintaining proper documentation in electronic health record (EHR) systems Provide specialized care for patients with dementia or Alzheimer's disease, including memory care techniques and behavioral management Assist with catheter care, wound care, and other medical procedures under supervision or as trained Help with light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe living environment Communicate effectively with clients and family members to address needs, preferences, and concerns while maintaining patient confidentiality in compliance with HIPAA regulations Skills Proven experience working with individuals with developmental disabilities or in assisted living environments Strong knowledge of home healthcare regulations and medical terminology Ability to perform patient transfer methods safely using equipment like Hoyer lifts or transfer belts Competency in taking vital signs and recognizing signs of medical emergencies requiring immediate attention Familiarity with EMR (Electronic Medical Record) systems and medical documentation standards Excellent patient care skills with a focus on geriatrics, hospice care, or long-term care settings Ability to handle heavy lifting safely and perform physically demanding tasks when necessary Certified in first aid and CPR, with ongoing commitment to maintaining these certifications Experience working within home health agencies or providing live-in care is highly valued Strong communication skills for effective client interaction and team collaboration Join us to deliver compassionate home health care that empowers individuals to live healthier, happier lives.
