Position Summary:
The Per Diem Direct Support Professional (DSP) provides hands-on, person-centered support to individuals with Intellectual and Developmental Disabilities (IDD) in home and community-based settings on an as-needed basis. This role supports daily living activities, skill development, health and safety monitoring, and community inclusion while promoting independence, dignity, and overall well-being. As a per diem employee, the DSP may be scheduled based on individual needs, program needs, staffing coverage, and employee availability. Hours may vary, and a specific number of hours or shifts is not guaranteed.
Position Scope:
The Per Diem DSP works directly with individuals served to implement individualized service plans (ISP/IPP) and behavior support strategies under the guidance of the Program Specialist and interdisciplinary team. This position focuses on direct care delivery and does not carry supervisory, hiring, or disciplinary authority. Per diem DSPs may provide coverage across available assignments based on organizational needs, individual service authorizations, employee qualifications, and availability. To maintain active per diem status, employees are expected to maintain sufficient availability and work at least one (1) shift per calendar month when an appropriate shift is offered and available. Failure to meet this requirement may result in review of the employee's per diem status, subject to applicable law and company policy.
Key Responsibilities:
The DSP provides consistent, respectful, and person-centered support in accordance with each individual's care plan, promoting safety, independence, dignity, and overall well-being. Responsibilities include assisting with personal care and daily living activities, meal preparation and nutrition, medication reminders or assistance as authorized, mobility and transfers, behavioral and emotional support, structured routines, health and safety monitoring, community participation, transportation, and employment or vocational activities. The DSP maintains accurate documentation, participates in required meetings and trainings, supports household organization related to care needs, and provides additional assistance during staffing shortages or special program needs as assigned.
Regulatory Frameworks:
The DSP must comply with all applicable federal, state, and program-specific regulations, including Pennsylvania IDD waiver requirements, provider licensing standards, individual service authorizations, HIPAA, and all OneWell Health Care policies and procedures.
Confidentiality:
The DSP has access to protected health information (PHI) and sensitive personal data. Strict confidentiality must be maintained at all times in accordance with HIPAA, state privacy laws, and company confidentiality policies. Unauthorized disclosure may result in disciplinary action up to and including termination.
Documentation Requirements:
Required documentation includes daily service notes with GPS verification, behavioral observations, incident and special incident reports, attendance and shift verification, and any additional records required to demonstrate compliance with care plans and regulatory standards. All documentation must be accurate, complete, and submitted within required timeframes.
Education:
High school diploma or equivalent.
Experience:
Preferred:
Prior experience in caregiving, home health, or direct support for individuals with IDD Experience working in community-based or residential support settings
Technical Skills:
Ability to use electronic documentation and EVV systems Basic computer skills, including email and mobile applications Ability to follow written care plans and documentation guidelines
Languages:
English proficiency (spoken and written) is required. Bilingual skills are a plus, depending on program and individual needs.
Driver License:
Valid driver's license may be required based on assignment. Must maintain an acceptable driving record and reliable transportation when driving is an essential job function.
Travel Requirement:
This is a community-based role requiring travel to individual homes, community locations, program sites, and appointments as assigned. Per diem scheduling is based on organizational needs and employee availability and may include days, evenings, weekends, holidays, and overnight shifts.
