Adult male Position Summary Diversified Home & Community Services LLC is seeking a compassionate, dependable, and qualified Enhanced Direct Support Professional to provide one-to-one Level 2 Enhanced In-Home and Community Support to an adult male in Warminster, Pennsylvania. The Enhanced Direct Support Professional will assist the individual with developing and maintaining independent living skills, participating in meaningful community activities, managing daily routines, and achieving the outcomes identified in his Individual Support Plan. The employee must provide person-centered support while protecting the individual's health, safety, dignity, privacy, choice, and independence. Essential Responsibilities Provide one-to-one enhanced support in the individual's home and community. Implement the individual's Individual Support Plan, Behavioral Support Plan, crisis plan, and other approved support strategies. Assist the individual with daily routines and activities of daily living, as authorized. Teach and reinforce independent living, communication, social, coping, decision-making, and community-safety skills. Support the individual with planning and participating in preferred community activities. Assist with scheduling and attending appointments, shopping, recreation, and other community activities. Provide behavioral and emotional support using positive, person-centered, and trauma-informed approaches. Recognize potential triggers and early signs of behavioral or emotional escalation. Implement approved de-escalation, prevention, and crisis-support strategies. Encourage the use of functional communication and appropriate coping skills. Support the individual with developing healthy routines and increasing independence. Monitor and report changes in the individual's health, behavior, functioning, or safety. Maintain professional boundaries and protect the individual's confidentiality. Complete accurate service notes, electronic visit verification, incident reports, data collection, and other required documentation within established timeframes. Communicate significant concerns promptly to the supervisor and appropriate members of the support team. Transport or accompany the individual in the community when authorized. Follow all DHCS policies, ODP requirements, mandated-reporting requirements, and applicable state and federal regulations. Participate in supervision, staff meetings, training, and competency evaluations. Perform other participant-specific responsibilities identified in the Individual Support Plan. Required Education Applicants must possess a bachelor's degree from an accredited college or university in one of the following ODP-recognized fields: Psychology Education Special Education Counseling Social Work Gerontology Therapeutic Recreation Registered Behavioral Technician (RBT) CNA A copy of the applicant's diploma and official or unofficial transcript must be provided for verification. Additional Qualifications Must be at least 18 years old. Experience supporting adults with intellectual disabilities, autism, behavioral health needs, or dual diagnoses is strongly preferred. Must demonstrate professionalism, patience, dependability, sound judgment, and effective communication skills. Must be able to follow an Individual Support Plan and Behavioral Support Plan consistently. Must be able to complete objective, accurate, and timely service documentation. Must successfully complete all required criminal-history, exclusion, and background screenings. Must possess a valid driver's license and have access to reliable transportation. If using a personal vehicle for services, the vehicle must have current registration, inspection, and automobile insurance. Must complete required ODP, agency, participant-specific, incident-management, health-and-safety, and medication-administration training, when applicable. CPR and First Aid certification is required or must be obtained within the agency's designated timeframe. Must be able to use an electronic documentation and electronic visit verification system. Must maintain confidentiality and comply with HIPAA requirements. Preferred Qualifications Previous experience providing enhanced in-home or community-based support. Experience implementing positive behavioral-support strategies. Experience supporting adults with complex behavioral, emotional, or adaptive needs. NADD Direct Support Professional Certification or willingness to pursue NADD certification. Training in trauma-informed care, de-escalation, crisis prevention, or applied behavioral-support strategies. Work Schedule This is a daytime position. The selected employee must be available to work the participant's authorized schedule. The schedule may include weekday daytime hours and community-based activities based on the participant's preferences, needs, and Individual Support Plan. Work Environment Services will be delivered in the individual's private home and in community locations throughout Warminster and surrounding areas. The employee may be required to travel between community locations, accompany the individual on outings, and respond appropriately to changing behavioral or environmental conditions. Pay:
