Responsibilities Assist clients with Activities of Daily Living (ADLs), including personal hygiene, grooming, dressing, and mobility support using manual handling techniques such as Hoyer lifts and patient transfer methods. Monitor and observe patients' health status by taking vital signs, recording care plans, and reporting any changes or concerns promptly. Support medication administration in accordance with HIPAA regulations and care plans, ensuring accuracy and safety at all times. Prepare nutritious meals and assist with meal planning to meet individual dietary needs, including specialized diets for dementia or Alzheimer's care. Maintain a clean and safe environment by performing light cleaning duties and ensuring proper infection control practices are followed. Provide transportation assistance for appointments or community outings while ensuring client safety during travel. Document all interactions accurately, communicate effectively with clients and team members, and uphold residents' rights at all times. Qualifications Prior experience working in group homes, assisted living facilities, nursing homes, or home health settings is highly preferred. Knowledge of patient observation techniques, medication administration, and vital signs monitoring is essential. Experience working with individuals with autism, dementia, Alzheimer's disease, or other neurodevelopmental disorders is a strong plus. Ability to perform heavy lifting and manual handling safely; familiarity with equipment such as Hoyer lifts is advantageous. Certification in CPR and First Aid is required; additional training in hospice care or memory care is beneficial. Strong communication skills to effectively support client needs and collaborate with team members while respecting client confidentiality under HIPAA guidelines.
