Job Summary Provide direct support to individuals with intellectual disabilities and/or autism in their homes and in the community. Assist individuals with building and maintaining skills needed to become more independent and work toward the goals identified in their Individual Support Plan (ISP). Responsibilities Support individuals with daily living and independent living skills. Assist with personal hygiene and self-care as needed. Support meal planning, cooking, and kitchen safety. Assist with cleaning, laundry, household organization, and other home skills. Support grocery shopping, budgeting and money management. Provide transportation and support individuals with accessing the community. Encourage participation in community activities based on the individual's interests Help individuals build communication and social skills. Support decision-making, problem-solving, and self-advocacy. Provide verbal prompts, reminders, modeling, supervision, or other assistance based on the individual's needs and ISP. Encourage individuals to complete tasks as independently as possible instead of completing tasks for them. Support individuals with appointments, schedules, and daily routines when identified in their ISP. Follow health and safety needs, behavioral supports, and other instructions identified in the individual's ISP. Respect each individual's rights, privacy, dignity, preferences, and choices. Observe and report changes or concerns involving an individual's health. Complete accurate service notes after each shift documenting activities. Follow all applicable ODP requirements, agency policies, training requirements, and incident-reporting procedures.
