Full time roles Mon thru Fri As a Patient Care Coordinator you will: Build meaningful relationships with your patient. Provide attention to the patient's non-medical needs, including companionship and social engagement, in a warm, empathetic manner Provide personal care to your patient (bathing, dressing, transfers, toileting, etc.) as needed Assist your patient with light housekeeping, meal preparation, laundry, linen changes, and medication reminders, as needed Supervise your patient to ensure their safety Establish communication and a professional relationship with your patient, their family members, and co-workers Provide reliable care by being punctual and consistently covering shifts Follow the care plan, as provided by the Director of Nursing, and documenting and communicating any observed changes.
