Responsibilities Assist clients with Activities of Daily Living (ADLs), including personal hygiene, dressing, grooming, and mobility support Monitor patients' health by taking vital signs, observing behaviors, and reporting changes in condition Administer medications following care plans and HIPAA confidentiality standards Support clients with meal preparation, feeding, and nutritional needs in accordance with dietary guidelines Utilize patient transfer methods such as Hoyer lifts and other manual handling techniques to ensure safe mobility Maintain accurate documentation of care activities, patient observations, and incident reports Provide transportation assistance for appointments or community outings while ensuring safety at all times Requirements Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred Knowledge of dementia care, Alzheimer's care, memory care, and hospice support is advantageous Familiarity with patient observation techniques and monitoring vital signs is essential Ability to perform light housekeeping tasks such as cleaning and maintaining client spaces Experience working with individuals with autism or developmental disabilities is a plus Certification in CPR and First Aid is required; additional certifications related to healthcare or caregiving are beneficial Strong communication skills for effective client interaction and team collaboration Join us if you're passionate about providing exceptional patient-centered care!
