Responsibilities Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and toileting, ensuring their comfort and dignity. Monitor patients' health status by observing vital signs, patient behavior, and overall condition; promptly report any concerns to healthcare professionals. Support medication administration following care plans and HIPAA regulations to maintain confidentiality and safety. Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption as needed. Facilitate patient transfers using manual handling techniques or equipment such as Hoyer lifts, ensuring safe mobility. Maintain a clean and safe environment by performing light cleaning duties and adhering to infection control protocols. Document care activities accurately in compliance with facility policies and resident's rights, including progress notes and incident reports. Skills Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly desirable. Strong patient observation skills with the ability to monitor vital signs and recognize changes in condition. Knowledge of dementia care, Alzheimer's care, memory care, hospice care, and neurodevelopmental disorders such as autism is preferred. Proficiency in patient transfer methods, manual handling, CPR, first aid, and basic life support (BLS). Ability to assist with ADLs efficiently while respecting individual preferences and privacy. Excellent communication skills for engaging with clients, families, and healthcare team members effectively. Familiarity with HIPAA regulations to ensure confidentiality of patient information at all times.
