Responsibilities Assist clients with personal care needs including bathing, grooming, dressing, and toileting to ensure comfort and hygiene. Support residents with mobility and transfer methods using equipment such as Hoyer lifts and patient transfer techniques. Monitor patient vital signs, observe changes in health status, and report findings accurately to healthcare professionals. Prepare nutritious meals tailored to individual dietary requirements and assist with meal consumption when necessary. Help clients with medication administration following prescribed care plans while adhering to HIPAA regulations for privacy and confidentiality. Maintain a clean, safe environment by performing light cleaning duties and ensuring infection control protocols are followed. Provide transportation assistance for medical appointments, errands, or community activities to promote social engagement and independence. Experience Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred. Demonstrated ability to perform patient observation, monitor vital signs, and assist with medication administration. Experience working with individuals with dementia, Alzheimer's disease, autism spectrum disorders, developmental disabilities, or neurodevelopmental disorders is advantageous. Knowledge of care plans, resident rights, HIPAA compliance, manual handling techniques including heavy lifting and patient transfers. Certification in CPR and First Aid is required; additional training in hospice care or geriatrics is a plus. Familiarity with assisting clients with ADLs (Activities of Daily Living), personal transportation assistance, meal preparation, and home & community care experience enhances your suitability for this role.
