Job Overview We are seeking a compassionate and dedicated Personal Care Assistant to provide exceptional support and care to individuals in various settings, including home health, assisted living, nursing homes, and group homes. Responsibilities Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using manual handling techniques like Hoyer lifts and patient transfer methods. Monitor patients' vital signs, observe changes in health status, and report findings accurately to healthcare professionals. Support medication administration following HIPAA guidelines and care plans established by healthcare providers. Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary. Maintain a clean and safe environment by performing light cleaning duties and ensuring infection control protocols are followed. Provide emotional support and companionship to clients, fostering a positive environment that respects residents' rights and dignity. Assist with transportation for appointments or community outings using personal transportation assistance or specialized equipment. Document patient observations, care activities, and communication with healthcare teams diligently to ensure continuity of care. Qualifications Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred. Knowledge of patient observation techniques, medication administration, vital signs monitoring, and emergency procedures such as first aid and CPR. Experience working with individuals with autism, dementia (including Alzheimer's care), neurodevelopmental disorders, or developmental disabilities is advantageous. Familiarity with care plans, resident's rights policies, HIPAA regulations, manual handling equipment like Hoyer lifts, and patient transfer methods. Ability to perform heavy lifting safely and assist with patient transfers while maintaining proper body mechanics. Strong communication skills for client interaction and teamwork within multidisciplinary healthcare settings. Certification in first aid and CPR is required; additional certifications related to geriatrics or hospice care are a plus. Demonstrated commitment to providing compassionate patient personal care assistance in home & community care settings.
