Job Description

Job Summary Join our dedicated healthcare team as a Home Health Aide, where you will play a vital role in providing compassionate, personalized care to individuals in their homes. This position offers an exciting opportunity to support clients with a variety of health and daily living needs, promoting independence and enhancing quality of life. You will work closely with patients, families, and healthcare professionals to deliver high-quality home health care assistance in a respectful and supportive environment. Responsibilities Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using patient transfer methods and equipment such as Hoyer lifts Monitor and observe patient conditions, recording vital signs accurately and reporting any changes to healthcare providers promptly Administer medications following medical instructions while maintaining strict adherence to home healthcare regulations and HIPAA confidentiality standards Support patients with medical procedures such as catheter care, wound care, and basic first aid or CPR when necessary Document patient care activities thoroughly using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems, ensuring accurate medical documentation and records management Provide companionship and emotional support to clients with dementia or Alzheimer's disease, creating a safe and comforting environment for memory care or hospice care needs Assist with meal preparation, light housekeeping, laundry, cleaning, and maintaining a safe home environment to promote overall well-being Qualifications Proven experience working as a Home Health Aide or in assisted living, nursing home, or long-term care settings; experience working with individuals with disabilities or developmental disabilities is highly valued Knowledge of medical terminology, patient transfer methods, medication administration, and home health care regulations Certification in First Aid and Basic Life Support (BLS), including CPR training; additional certifications such as hospice care or dementia care are advantageous Ability to work independently while following detailed care plans; excellent communication skills for client interaction and team collaboration Physical ability to perform heavy lifting when necessary and assist with patient transfers using equipment like Hoyer lifts or patient transfer methods Familiarity with EMR/EHR systems for documentation; understanding of HIPAA regulations to maintain patient confidentiality at all times Compassionate attitude with a strong commitment to providing exceptional senior care and home & community care assistance Join us if you're passionate about making a positive impact through dedicated caregiving!
