- Be part of a team that supports your development and success What You'll Do Provide compassionate, hands-on care that supports patient comfort, safety, and independence: Assist patients with daily living activities, including bathing, dressing, grooming, and toileting Support mobility, transfers, and basic exercises in alignment with the care plan Help maintain a clean, safe, and comfortable home environment Prepare meals and assist with feeding as needed Take and record vital signs when required and trained to do so Assist with medication self-administration as directed Observe and report changes in patient condition to the clinical team Document care provided accurately and timely within the EMR system Participate in team meetings, trainings, and care coordination activities Provide compassionate support to patients and families during each visit What We're Looking For We're seeking individuals who are dependable, compassionate, and comfortable working independently in a home-based setting: Meets state and federal requirements for Home Health Aide certification or training At least 18 years of age Ability to read, follow care plans, and document services provided Current CPR certification Valid driver's license, reliable transportation, and auto insurance Ability to work independently with strong time management skills Demonstrates empathy, patience, and professionalism in all interactions What Sets You Apart Experience providing personal care in a home health, hospice, or caregiving setting Strong observation skills with the ability to recognize and report changes Compassionate, patient-centered mindset Reliable, accountable, and self-motivated Positive attitude and strong communication skills At VitalCaring, your care matters.
