• Monitor and assist individuals with daily activities
• Develop and implement personalized care plans
• Provide direct support and companionship
• Observe and document changes in patients' conditions
• Assist with vital signs monitoring
• Support individuals with developmental disabilities in various tasks
• Offer memory care assistance as needed Experience
• Prior experience in patient monitoring and care is preferred
• Familiarity with creating care plans
• Ability to work compassionately with individuals with disabilities
