Job Overview The Community Coaching - Direct Support Professional (DSP) provides direct care, support, and skill development to individuals with intellectual and developmental disabilities (IDD) served by Hope and Harmony Group Home. This role focuses on helping individuals achieve greater independence, community inclusion, and personal growth through structured activities, coaching, and daily living support in the community. The DSP is responsible for supporting each individual's person-centered plan, promoting dignity and respect, ensuring safety, and fostering meaningful participation in daily life. Duties Provide individualized community coaching that supports each person's goals, interests, and abilities as outlined in their Individual Service Plan (ISP). Teach and reinforce life skills such as personal care, money management, time management, and decision-making. Support individuals in community integration activities, including recreation, volunteering, church involvement, social clubs, and civic events. Encourage participation in educational and enrichment activities, such as classes, workshops, or skill-based programs that promote independence. Assist with communication and social skill development, including initiating conversations, maintaining friendships, and resolving conflicts appropriately. Model and teach appropriate community behavior and safety awareness (e.g., stranger safety, emergency preparedness, and respecting public spaces). Support individuals in developing healthy lifestyles—including meal planning, exercise routines, and stress management techniques. Help individuals explore personal interests and hobbies, fostering creativity and confidence. Encourage individuals to engage in self-advocacy and express preferences and choices in daily and community activities. Foster community belonging by connecting individuals with local organizations, volunteer opportunities, and peer support networks. Use positive reinforcement and strength-based approaches to help individuals achieve personal milestones and maintain motivation. Assist individuals with technology use, including smartphones, tablets, or computers, to enhance independence, communication, and community connection. Evaluate and report progress on coaching goals regularly, making recommendations to supervisors or case managers for adjustments when needed. Collaborate with the residential team to ensure that coaching goals are consistent with in-home support and overall person-centered outcomes. Documentation and Reporting Accurately document daily progress notes, community activities, and outcomes according to program and Medicaid requirements. Complete attendance, incident, and behavior reports as required. Communicate promptly with supervisors regarding changes in individual needs, health, or behaviors. Maintain confidentiality in accordance with HIPAA and agency policies. Safety and Compliance Follow all Hope and Harmony Group Home policies and procedures, including health, safety, and emergency protocols. Ensure proper supervision ratios are maintained during community outings and all activities. Transport individuals safely using agency or personal vehicle (as approved) while following all traffic laws and safety standards. Participate in required trainings, certifications, and meetings as directed by management.

Qualifications Education and Experience:

High School Diploma or GED required. Minimum of one (2) year experience working with individuals with intellectual and developmental disabilities preferred. Experience in community-based support, coaching, or day support programs strongly desired. Excellent interpersonal skills, with the ability to communicate effectively and empathetically with individuals, families, and colleagues. Certifications (Preferred or Required): CPR and First Aid Certification Medication Administration Training Behavior Intervention Training (e.g., TOVA) Valid driver's license with acceptable driving record and reliable transportation Physical Requirements Ability to stand, walk, lift up to 50 lbs., bend, and assist individuals with mobility as needed. Ability to drive, accompany individuals in the community, and respond quickly in emergencies.
