Duties Assist clients with activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility support using proper patient transfer methods and manual handling techniques like Hoyer lifts. Monitor patients' vital signs and observe for any changes in health or behavior, documenting findings accurately in care plans. Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary. Administer medications following prescribed instructions while adhering to HIPAA regulations to ensure patient confidentiality. Support residents with memory care needs, including dementia and Alzheimer's care, providing personalized support aligned with their care plans. Maintain a clean and safe environment by performing light cleaning duties and ensuring infection control protocols are followed. Provide emotional support through compassionate communication, fostering trust and comfort during interactions with clients and their families. Skills Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred. Strong patient observation skills to detect changes in condition or behavior promptly. Knowledge of patient care techniques including patient transfer methods, medication administration, vital signs measurement, CPR, first aid, and HIPAA compliance. Experience supporting individuals with developmental disabilities, neurodevelopmental disorders such as autism, or dementia-related conditions like Alzheimer's disease. Ability to perform heavy lifting safely using manual handling equipment such as Hoyer lifts when required. Excellent communication skills for effective client interaction and collaboration with healthcare teams. Familiarity with care plans, medication management, patient monitoring, and documentation practices in home & community care settings.
