This hybrid role is ideal for an experienced home care professional who enjoys mentoring caregivers, building relationships with clients and families, and helping ensure exceptional care is delivered every day. You'll provide hands-on client care, coach and support caregivers, assist with new client assessments and introductions, and provide leadership in the field by supporting new client starts, assisting with care transitions, and stepping in to ensure clients continue receiving exceptional care whenever additional support is needed. While this is not a scheduling position, you'll become familiar with our office processes and scheduling systems to provide support when needed and work collaboratively with our administrative team.

Position Details Part-Time:

Approximately 25-30 hours per week Flexible schedule with availability across seven days to support client and business needs Hybrid position with both office and field responsibilities Reports to: Owners & Administrative Team Must have own vehicle Qualifications 3-5 years of professional home care experience CNA, HCA, or equivalent experience preferred Leadership or mentoring experience is a plus Excellent communication and organizational skills Reliable transportation and valid driver's license Comfortable using technology, including Outlook, Teams, Microsoft Office, and WellSky (or willing to learn) Key Responsibilities Client Care Provide exceptional personal care and companionship. Conduct new client assessments and introductions. Build strong relationships with clients and their families. Support care plan implementation and follow-up visits. Provide leadership in the field by supporting new client starts, assisting with care transitions, and ensuring clients continue receiving exceptional care whenever additional support is needed. Caregiver Leadership Mentor, coach, and support caregivers in the field. Assist with caregiver onboarding and first client visits. Promote company standards and best practices. Serve as a trusted resource and role model for the caregiving team. Operational Support Learn office operations and scheduling processes to provide support when needed. Communicate client updates and caregiver concerns with the office. Assist with new client launches, orientations, and other operational needs. Why Join Our Team?
