In this energetic role, you will provide compassionate, hands-on support to help clients achieve their highest level of independence and quality of life. Duties Assist clients with activities of daily living (ADLs), including personal hygiene, dressing, grooming, and mobility support using manual handling techniques like Hoyer lifts and patient transfer methods. Monitor patients' vital signs and observe for changes in health or behavior, documenting findings accurately while adhering to HIPAA regulations to ensure privacy and confidentiality. Prepare nutritious meals tailored to individual dietary needs, ensuring proper food handling and cleanliness standards are maintained. Support clients during outings or appointments by providing transportation assistance and ensuring their safety throughout community activities. Implement care plans designed by healthcare professionals, including medication administration, patient monitoring, dementia care, Alzheimer's care, memory care, and supporting individuals with neurodevelopmental disorders such as autism. Maintain a clean and safe environment by performing routine cleaning tasks, laundry, and organizing client spaces in compliance with health standards. Communicate effectively with clients' families and healthcare teams to update on progress or concerns while respecting client rights under HIPAA guidelines. Requirements Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred. Knowledge of patient care techniques including patient observation, vital signs measurement, medication administration, first aid & CPR certification. Experience supporting individuals with dementia, Alzheimer's disease, autism spectrum disorder (ASD), developmental disabilities or neurodevelopmental disorders is a strong plus. Ability to perform heavy lifting or manual handling tasks safely using appropriate patient transfer methods such as Hoyer lifts. Strong communication skills for effective client interaction and collaboration with healthcare teams. Demonstrated patience and compassion when working with vulnerable populations requiring personalized support. Valid certifications in first aid & CPR; additional certifications related to hospice care or geriatrics are advantageous.
