Job Summary:
An LVN/LPN (Licensed Vocational Nurse/Licensed Practical Nurse) in a Long-Term Care (LTC) facility provides direct nursing care to residents who require ongoing medical and personal support. They administer medications, monitor residents' health, assist with daily living activities, and work under the supervision of a Registered Nurse (RN) or physician to ensure high-quality, compassionate care.
Roles & Responsibilities:
Provide direct nursing care to long-term care residents. Administer prescribed medications and treatments safely and accurately. Monitor and document residents' vital signs, symptoms, and overall condition. Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, feeding, and mobility. Perform wound care, dressing changes, and other basic nursing procedures. Observe and promptly report changes in residents' conditions to the RN or physician. Maintain accurate and timely medical records and documentation. Assist with admissions, transfers, and discharges of residents. Educate residents and their families on medications, treatments, and care plans. LN01
Required Qualifications:
