Summary Sunshine Community Health Center is seeking a Licensed Practical Nurse (LPN I, II, or III) to join our interdisciplinary care team. The LPN provides direct clinical support and specialty nursing services within the scope established by the State of Alaska Board of Nursing. Working within a patient-centered medical home, the LPN uses a collaborative, team-based, and trauma-sensitive approach to provide compassionate, effective care. This position also supports continuity of care, quality improvement efforts, and the ongoing development of clinical support staff. Essential Duties Provide direct clinical nursing support to patients within the LPN scope of practice and State of Alaska Board of Nursing regulations. Collaborate with providers and other members of the healthcare team to develop and implement patient care plans. Provide specialty nursing services based on patient needs. Perform clinical procedures including patient assessments, wound care, injections, specimen collection, and other duties within the LPN scope of practice. Provide patient education regarding medical conditions, treatment plans, medications, and preventive care. Maintain accurate and timely documentation in the electronic health record. Participate in interdisciplinary care planning and promote continuity of care. Support a trauma-sensitive, patient-centered approach to care. Participate in quality improvement activities and implementation of established quality standards. Collaborate with providers, nurses, clinical support staff, and the Quality team to identify opportunities to improve patient care and clinical processes. Maintain required licensure, certifications, clinical competencies, and annual competency requirements. Promote a safe, professional, respectful, and collaborative clinical environment. Perform other related duties as assigned.

Level-Specific Responsibilities:

LPN I:

No formal supervisory responsibilities.

LPN II:

Co-training responsibilities with the Clinical Floor Supervisor and in collaboration with the Quality Manager.

LPN III:

Co-training and additional supervisory and leadership responsibilities in collaboration with the Chief Nursing Officer. Education/Experience Graduation from an accredited practical nursing program. Current Licensed Practical Nurse licensure in the State of Alaska. Current Basic Life Support (BLS) certification required. Knowledge of State of Alaska Board of Nursing regulations. Experience providing direct clinical support and specialty nursing services. Strong interpersonal and communication skills. Ability to work collaboratively as part of an interdisciplinary healthcare team. Demonstrated clinical judgment, critical-thinking, problem-solving, and time-management skills. Experience with or ability to effectively use electronic health record systems. Ability to provide compassionate, culturally responsive, and patient-centered care. Leadership and training abilities appropriate to the LPN level for which the applicant is being considered.
