Monday-Friday 2:30pm-11:00pm/Weekends 6:30pm-7:00am with baylor planJob Summary:
Birmingham Rehabilitation and Wellness Center is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team of dedicated healthcare professionals. As an integral part of our interdisciplinary team, the LPN will provide high-quality patient care and support in a dynamic and fast-paced rehabilitation setting.Responsibilities:
Provide direct patient care under the supervision of a registered nurse, including administering medications, wound care, and performing vital signs Develop and implement individualized care plans in collaboration with the interdisciplinary team Maintain accurate and up-to-date patient records and documentation Participate in patient education and discharge planning to ensure a smooth transition to home or community-based care Collaborate with physical, occupational, and speech therapists to ensure comprehensive patient rehabilitation Maintain a safe and clean environment for patients, staff, and visitorsRequirements:
Current LPN licensure in theState of Alabama CPR Certification What We Offer:
