Job Summary Join our dedicated healthcare team as a Night Shift LPN/RN working from 6 p.m. to 6 a.m., where your expertise will make a meaningful difference in patient care during overnight hours. In this vital role, you will provide comprehensive nursing services, ensuring the safety, comfort, and well-being of our residents and patients. Your proactive approach and clinical skills will support a smooth night operation, fostering a calm and caring environment for those we serve. This position offers an exciting opportunity to apply your diverse nursing experience in a dynamic setting that values compassion, professionalism, and teamwork. Shift differential offered to full time night shift employees. Responsibilities Deliver high-quality patient care through thorough assessments, vital signs monitoring, and physical examinations during overnight shifts Administer medications and injections accurately, including IV infusions, adhering to all safety protocols and HIPAA regulations Manage complex cases such as acute pain management, dementia care—including Alzheimer's and memory care—and infectious disease treatment Operate EMR (Electronic Medical Records) systems like Point Click Care for precise medical documentation and patient records management Conduct patient assessments for triage purposes, prioritizing urgent needs in emergency or acute care situations Assist with medical procedures such as catheterization, tube feeding, sterile techniques, and wound suturing when necessary Collaborate with interdisciplinary teams on discharge planning, case management, and post-acute care strategies to ensure seamless patient transitions Maintain infection control standards and ensure compliance with all healthcare regulations including HIPAA and NCQA standards Support patients with developmental disabilities or those requiring specialized memory or behavioral health care in a compassionate manner Experience Prior nursing roles in long-term care facilities like nursing homes or assisted living communities emphasizing geriatrics and dementia care Hands-on experience with medical-surgical nursing, home & community care, or outpatient clinics involving patient assessment and medical documentation Familiarity with EMR/EHR systems such as Point Click Care Knowledge of ICD coding (ICD-9/ICD-10), CPT coding for procedures, and diagnostic evaluation techniques Experience administering IV infusions, injections, phlebotomy procedures including venipuncture and catheter care Strong understanding of physiology, anatomy, pharmacology related to pain management.
