Job Description

Mercy is seeking a dedicated Primary Care Nurse RN/LPN to deliver compassionate, patient-centered care in a hybrid clinic/remote model. In this role, you will provide triage, chronic disease management, medication administration, care coordination, and patient education for diverse populations. You will collaborate closely with physicians and interdisciplinary teams, utilize electronic health records, and support telehealth visits. Responsibilities Provide primary care nursing for diverse patient populations in a clinic and telehealth setting Perform patient triage, assessments, and vital sign monitoring Administer medications, immunizations, and treatments per provider orders Support chronic disease management and care coordination across the continuum Educate patients and families on health conditions, medications, and self-care Document care accurately in electronic health records (EHR) systems Collaborate with physicians and interdisciplinary team members to develop care plans Assist with telehealth visits, remote monitoring, and follow-up outreach Adhere to clinical protocols, infection control, and regulatory standards Participate in quality improvement initiatives and ongoing professional development Required Skills Patient assessment Clinical triage Medication administration Chronic disease management Telehealth support Electronic Health Records (EHR) documentation Care coordination Patient and family education Infection control practices Interdisciplinary teamwork
