Qualifications:

Valid and active LPN license At least one year of recent experience providing care in a home health setting Demonstrated proficiency in HomeCare HomeBase (HCHB) EMR Exceptional clinical assessment and documentation capabilities Strong organizational and time-management skills to handle independent caseloads efficiently Reliable transportation to travel to patient homes

Responsibilities:
