This full-time contract position empowers you to promote independence, deliver patient education, and foster recoveryall while providing compassionate clinical care in collaboration with a supportive interdisciplinary team.

Schedule and Setting:

Monday Friday, 8:00 AM 5:00 PM (with flexibility for patient needs) Case load: 2532 points weekly (about 35 home visits/day) Field-based, serving an assigned territory Home Care Home Base (HCHB) documentation required

Key Responsibilities:

Conduct skilled assessments and implement individualized care plans for home health patients Deliver medication administration, wound care, IV therapy, and clinical interventions as ordered Monitor and promptly communicate changes in patient condition to supervising RN or physician Educate patients and caregivers on diagnoses, medication management, and self-care Chart all care using Home Care Home Base in alignment with compliance standards Collaborate with the healthcare team to coordinate and review patient care Adhere to infection control guidelines and ensure ongoing patient safety Participate in case conferences, quality initiatives, and required training programs

Required Qualifications:

Active, unrestricted LPN license in AR At least 1 year of experience in home health, hospice, or similar community-based care Home Care Home Base (HCHB) proficiency Valid drivers license and reliable vehicle with insurance Current BLS certification Excellent clinical judgment, organization, and communication abilities Ability to excel independently while managing a diverse and dynamic caseload
