Trinity Village Assisted Living and Nursing Home Facilities
NIGHT SHIFT
And Day ShiftAVERAGE MAXIMUM OF 20 RESIDENTS
(EXCEPTUNDER EMERGENT SITUATIONS
) Conduct and manage treatment orders for residents in accordance with current Federal, State, and Local regulations that govern the facility, to ensure that quality care is promoted at all times. This position is a safety sensitive position as defined by applicable state law.QUALIFICATIONS
- Must be a Licensed Practical Nurse in good standing and currently licensed by the State.
- Experience in Long-Term Care preferred.

PHYSICAL REQUIREMENTS OF JOB
- Walking, reaching, bending, lifting, grasping, pushing/pulling, fine hand coordination, ability to understand and follow written/oral instructions and directions; ability to communicate with residents, families, outside agencies and consultants, and the ability to remain calm under stress.
RESPONSIBILITIES
- Assuring Resident safety.
- Assist with the implementation of procedures for the administration and control of medication, and stocking of all nursing supplies/equipment.
- Assist with providing training to all shifts as necessary.
- Complete treatment orders in accordance with Physician orders.
- Assist with and/or complete skin audits as determined by supervisor.
- Be responsible for the safety of residents under his/her supervision.
- Identify safety hazards and initiate corrective action. Identify special nursing problems and emergent situations quickly and initiate immediate action.
- Communicate problems and needs with Nursing Supervisor/Designee.
- Report changes in residents; condition and take action as necessary.
- Provide emergency care to residents when necessary. Perform basic nursing skills, within LPN Scope of Practice.
- Participate in interdisciplinary team meetings if required by Supervisor.
- Provide input for Individualized Resident Care Plan.
- Observe all facility safety policies and procedures, infection control procedures, and other applicable policies and procedures.
- Read and become familiar with the Emergency Management Plan.

- Be available to provide Nursing needs for the facility.
