Job Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform care plans. Administer medications and injections accurately following prescribed protocols, including IV infusions when applicable. Assist with diagnostic evaluations such as specimen collection, processing, and basic medical imaging procedures. Support patient mobility and comfort through activities like catheter care, tube feeding, wound dressing, and aseptic techniques. Document all patient interactions thoroughly using Electronic Health Records (EHR) systems while maintaining strict compliance with HIPAA and clinical confidentiality policies. Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to each patient's needs. Provide education and support to patients and their families regarding health management, medication administration, and discharge planning. Requirements Valid state licensure as a Licensed Practical Nurse (LPN) with current certification in Basic Life Support (BLS) and CPR. Proven experience working in diverse healthcare environments such as long-term care facilities, hospitals, assisted living communities, or home care settings. Familiarity with EMR/EHR systems like Epic, Cerner, Athenahealth, or eClinicalWorks for accurate documentation. Experience with specialized areas including geriatrics, dementia care (Alzheimer's and memory care), hospice care, pediatrics, or critical care such as ICU or Level I/II trauma centers is highly desirable. Knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), medication administration protocols, and infection control practices. Ability to work effectively with individuals with disabilities or developmental challenges while demonstrating excellent caregiver communication skills. Strong organizational skills for intake assessments, discharge planning, case management activities, and documentation accuracy.
