Work Statement Services to be

Performed:

Administer physician-ordered injectable medications for mental health and substance use disorder treatment (e.g., long-acting injectable antipsychotics, injectable MAT such as naltrexone/buprenorphine formulations), within scope Monitor and record vital signs, including pre/post-injection monitoring per medication labeling (e.g., post-injection observation for medications with PIS risk); report changes in patient condition to the supervising provider. Provide patient/family education on health management and disease prevention. Document all encounters in Electronic Health Record. Comply with HIPAA and 42 CFR Part 2 Time for

Face-to-face, on-site only. Active unrestricted AZ license; Level 1 Fingerprint Clearance Card; current CPR/BLS.

For LPNs:

Practices under named supervising physician, available during all clinic hours.

Pay:

$30.00 - $45.00 per hour People with a criminal record are encouraged to apply

Education:

High school or equivalent (Preferred)

License/Certification:

AZ Level 1 Fingerprint Clearance card (Preferred)
