In this vital role, you will provide high-quality nursing services directly in patients' homes, promoting health, safety, and independence. Your expertise in various medical procedures, assessment techniques, and patient education will be essential to delivering exceptional home health services. Responsibilities Conduct comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations to determine care needs. Administer medications, injections, IV infusions, and perform catheterizations following strict protocols and HIPAA regulations. Manage wound care procedures such as suturing, dressing changes, and sterile technique applications to promote healing. Assist with complex medical tasks including tube feeding, ventilator management, and airway management in accordance with physician orders. Document all patient interactions accurately using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems like Epic or Cerner; ensure compliance with medical documentation standards. Coordinate discharge planning and case management activities to facilitate smooth transitions of care across settings. Provide education on disease management such as Alzheimer's care, dementia care, pain management, and medication administration to patients and their families. Skills Proven experience with Epic EMR systems or similar EHR platforms. Strong critical care background including ICU experience, Level I/II trauma center exposure, or hospital medicine is preferred. Knowledge of infectious disease protocols, hospice care standards, acute pain management techniques, and post-acute care practices. Skilled in performing venipuncture, phlebotomy, specimen collection/processing, and laboratory procedures with aseptic technique. Excellent patient assessment abilities combined with physiology knowledge to evaluate complex cases involving geriatrics or pediatrics. Ability to work effectively with individuals with disabilities or developmental delays; experience in assisted living or long-term care environments is a plus. Familiarity with medical terminology related to internal medicine, emergency medicine, occupational health, or neonatal ventilator management enhances your versatility.
