The LVN will provide high-quality patient care across various settings, including long-term care, assisted living, hospitals, and outpatient clinics. The ideal candidate will possess extensive experience with diverse patient populations, including geriatrics, pediatrics, and individuals with developmental disabilities or complex medical needs. Proficiency in EMR/EHR systems such as Epic, Cerner, and Athenahealth is highly desirable. This role requires a compassionate professional capable of performing comprehensive assessments, medication administration, IV infusion, and specialized procedures such as catheterization and sterile processing. The LVN will play a vital role in patient advocacy, documentation accuracy, and ensuring compliance with HIPAA and other regulatory standards. Responsibilities Conduct thorough patient assessments including vital signs, physical examinations, and physiological evaluations. Administer medications and injections accurately while adhering to safety protocols. Perform IV insertions, catheter care, tube feeding, and ventilator management as required. Document all patient interactions and procedures meticulously within EMR/EHR systems such as Epic, Cerner, or Athenahealth. Assist with diagnostic evaluations including laboratory specimen collection/processing and medical imaging coordination. Manage acute pain cases, dementia care (Alzheimer's and Memory Care), hospice support, and post-acute care planning. Provide specialized care for patients with infectious diseases, trauma (Level I & II Trauma Centers), ICU patients, neonatal ventilator management, and pediatric cases including

NICU/PICU.

Support discharge planning and case management activities to ensure seamless transition of care. Collaborate with multidisciplinary teams in settings such as emergency rooms, urgent care clinics, inpatient units, outpatient clinics, and home & community care environments. Maintain infection control standards using aseptic techniques during procedures like suturing or spinal taps. Educate patients on health coaching topics including nutrition, medication adherence (ICD/ICD-10 coding), and disease management strategies. Experience Proven experience working in diverse healthcare environments such as hospitals (including trauma centers), nursing homes, assisted living facilities, outpatient clinics, or home health settings. Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, Athenahealth; familiarity with

ICD-9/ICD-10

coding is preferred. Critical care experience including ICU or emergency medicine is highly valued. Experience managing complex cases involving acute pain management, infectious disease care, dialysis or wound suturing is advantageous. Background working with special populations such as pediatrics (including neonatal ventilator care), geriatrics (dementia & memory care), behavioral health patients or individuals with disabilities is desirable. Knowledge of anatomy & physiology principles essential for accurate patient assessment and diagnostic evaluation. Valid LVN license required along with current CPR/BLS certification; experience administering injections and phlebotomy is necessary.
