In this vital role, you will provide high-quality nursing care to diverse patient populations. Your expertise will help ensure patients receive personalized, efficient, and safe care while supporting their health and well-being. This position offers an exciting opportunity to make a meaningful difference in the lives of individuals with complex medical needs, including those with developmental disabilities, dementia, or recovering from acute injuries. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and detailed health histories using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems. Administer medications via injections, IV infusions, and other routes while adhering to HIPAA regulations and safety protocols. Manage specialized care procedures such as catheterization, tube feeding, wound suturing, and ventilator management for patients with acute or chronic conditions. Provide triage services by evaluating patient symptoms to determine urgency levels and appropriate interventions in urgent care or emergency room settings. Collaborate with multidisciplinary teams on discharge planning, case management, and coordination of post-acute or home & community care services. Support patients with dementia or Alzheimer's disease through memory care techniques and behavioral health strategies. Document all patient interactions accurately in medical records, ensuring compliance with CPT coding standards and
ICD-10/ICD-9
coding requirements for diagnoses. Qualifications Valid LVN license issued by the state licensing board with current certification to practice nursing. Proven experience in critical care environments such as ICU or Level I Trauma Centers is highly desirable. Demonstrated proficiency with EMR/EHR systems like Cerner, Epic, Athenahealth, or eClinicalWorks. Experience working with diverse populations including pediatrics, geriatrics, neonatal ventilator management, or individuals with disabilities is preferred. Strong knowledge of physiology, anatomy, pharmacology, infection control practices, and medical terminology. Ability to perform complex procedures such as phlebotomy, venipuncture, sterile processing techniques, and aseptic techniques confidently. Excellent communication skills for patient education on health coaching topics like nutrition or medication administration while maintaining HIPAA compliance.
