JOB DESCRIPTION SUMMARY
An LVN who specializes in the care of patients in the home, including the provision of skilled services and the provision of instruction to the patient and family related to the patient's conditions that ensures safe care and promotes self care. The LVN works under the supervision and direction of a Registered Nurse.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES 1.
Current license to practice as a Licensed Vocational Nurse in the state of California 2. Minimum of 1 year experience as a LVN in a clinical setting 3. Home Health experience preferred 4. Possession of a valid California Drivers license, an automobile and auto liability insurance
POSITION QUALIFICATIONS 1.
Works under the direction and supervision of the primary Registered Nurse on the case 2. Educates and instructs the patient, patient's family or staff as needed, according to that as allowed by the Licensed Vocational Nurse practice act 3. Maintains a scheduling calendar for primary patients 4. Plans and organizes workday efficiently 5. Reports to Nursing Supervisor / OPCS visits made, patient status and significant evens in a timely manner 6. Contacts and consults with Registered Nurse and Physician and documents significant findings 7. Treats patients and their families in accordance with the principles of Patient's rights 8. Presents self in a professional manner 9. Attends meetings, case conferences and in-services as required 10. Performs all nursing procedures and interventions according to Agency policies and procedures and according to acceptable standards of practice 11. Handles medical waste in a manner consistent with Agency policies and procedures and OSHA guidelines 12. Uses proper body mechanics 13. Assists RN with supervision of CHHA's as requested 14. Assists in orientation of other personnel as needed 15. Participates in chart audits and on committees as assigned 16. Documents in an appropriate, thorough and legible manner according to Medicare guidelines using appropriate forms 17. Completes verbal orders for changes in the Plan of Care 18. Demonstrates knowledge of Medicare guidelines for home care 19. Acts as the patient's advocate 20. Actively seeks additional information and / or training to provide optimal care to patients 21. Maintains adequate amounts of car supplies, does regular inventories and replaces items damaged or expired 22. Maintains inventory of patients supplies and orders additional supplies as needed 23. Regularly and appropriately submits charges for supplies used 24. Demonstrates the ability to work with others in a team approach to patient care 25. Reports for patient care assignments and meetings in a prompt manner 26. Accurately records visit times and mileage on the Daily Activity Record 27. Treats all patient information in a confidential and professional manner. 28. Performs duties consistent with the Vocational Nursing Practice Act.
