English Clinical documentation Medical terminology Full Job Description We are seeking a compassionate, organized, and motivated Licensed Vocational Nurse (LVN) to join our Enhanced Care Management (ECM) team serving IEHP members. In this role, the LVN will provide direct care coordination support for patients with complex medical, behavioral, and social needs while working closely with providers, case managers, Community Health Workers, and community-based organizations. The LVN will play a critical role in improving patient outcomes by assisting with care planning, medication education, chronic disease management, transitions of care, and addressing barriers related to social determinants of health. This position focuses on delivering patient-centered support, improving access to care, reducing avoidable hospitalizations, and promoting overall wellness for vulnerable populations. This is an excellent opportunity for an LVN passionate about community health, managed care, and making a meaningful impact in underserved communities. Duties & Responsibilities Conduct telephonic and in-person outreach to IEHP members enrolled in the Enhanced Care Management (ECM) program Assist with comprehensive health assessments and contribute to individualized care plans under the direction of the care team Provide patient education regarding chronic disease management, medications, preventive care, and healthy lifestyle practices Monitor and follow up with patients regarding medication adherence, treatment plans, referrals, and appointments Assist patients in navigating healthcare services including primary care, specialty care, behavioral health, home health, and community resources Support transitions of care following hospitalizations, emergency room visits, SNF stays, or other care settings Identify barriers to care including transportation, housing instability, food insecurity, and financial challenges, and connect members with appropriate community resources Collaborate with physicians, nurse practitioners, behavioral health providers, social workers, Community Health Workers (CHWs), and ECM care teams to coordinate patient care Maintain accurate and timely documentation in the electronic health record (EHR) while ensuring HIPAA compliance Participate in interdisciplinary team meetings, case conferences, and quality improvement initiatives Assist with obtaining medical records, authorizations, referrals, and care coordination activities Conduct patient follow-up calls and wellness checks to ensure continuity of care and engagement in treatment plans Educate patients and caregivers regarding available IEHP benefits and ECM program services Support outreach events and community engagement activities as needed Qualifications Current and active California Licensed Vocational Nurse (LVN) license required Minimum of 1-2 years of clinical experience preferred Experience working with Medi-Cal, managed care, or IEHP populations preferred Experience in care coordination, case management, primary care, home health, behavioral health, or community health strongly preferred Familiarity with Enhanced Care Management (ECM) and CalAIM programs preferred Strong understanding of chronic disease management and social determinants of health Knowledge of medical terminology, medication management, and healthcare systems Ability to communicate effectively with diverse patient populations and multidisciplinary healthcare teams Strong organizational skills and ability to manage multiple patient cases simultaneously Experience documenting in electronic health record (EHR) systems Bilingual English/ Spanish preferred Valid driver's license and reliable transportation preferred for community-based visits Work Location Hybrid field and office-based position Riverside County / San Bernardino County service areas preferred
